New Hire Enrollment
Your New Hire Enrollment period begins on the first day of the month following your date of hire. This is your chance to review your personal situation and consider your HDT plan choices. Outside of this enrollment period, you may only make benefit changes during the annual Open Enrollment period, or if you experience a qualified life event (e.g., birth, adoption, marriage, divorce, loss of spouse/partner health coverage). Elections can be made online via the HDT Intranet home page.

Medical coverage through Meritain Health
- Three plans to choose from (1 PPO Plan and 2 High Deductible Health Plans (HDHP)) on the Aetna network
- Health and Wellness Resources – online and telephonic resources to help you get, and stay, healthy
- Customer Service – you can contact Meritain Health by phone (800-925-2272) or online as www.meritainhealth.com whenever you need assistance
- Access to Healthcare Bluebook - a premier cost transparency tool that also will give you data on the best outcomes by procedure. To access Healthcare Bluebook, please log into your member portal at www.meritainhealth.com. Then, click on the Healthcare Bluebook tile near the bottom of the page
How to Search for an In-Network Provider
- Go to www.meritainhealth.com
- Hover over “Resources” and select “For members”
- Scroll down to the “Provider network finder” and select the Aetna network
Prescription Drug Coverage through Meritain and CVS
- Visit locations such as CVS, Target, Walgreens, Costco, Walmart, and more to pick up your prescriptions
- Mail order prescriptions will be provided through CVS
- 4-Tier Prescription drug coverage (Generic, Preferred, Brand, and Specialty)
- You are on the Value Formulary, which covers all generic medications and listed brand medications

Please note: For your convenience, there is one ID card for all your medical and prescription drug services
Dental Coverage through Delta Dental
- DPPO Network – your choice of visiting both in- and out-of-network providers
- Annual preventative services covered at 100%, not subject to the annual deductible
- Orthodontia coverage for children
Vision coverage will be administered by EyeMed, and will utilize the Insight network
Unum is the vendor for your Basic Life AD&D, Short Term Disability (STD), Long Term Disability (LTD), Voluntary Life AD&D, Accident, Critical Illness, and Hospital Indemnity coverages
Basic Life AD&D
We offer all employees a benefit of 2x your base annual earnings, up to a maximum of $300,000, at no cost to you
STD
We offer a STD plan to all employees, at no cost to you
LTD
We offer an LTD plan to all employees, at no cost to you
Voluntary Life/AD&D
We offer additional life insurance with Voluntary Life/AD&D plans to all employees that you pay for through payroll deductions. Coverage is available for yourself and your eligible spouse and child(ren).
